Please note that it is the player's responsibility to provide a valid, current address on their entry form. Any substitutions must be made at check-in and the substitute's form must state the original entrant's name. Entry fees include green fees, a riding cart, a prize fund, and a "19th Hole" fund. Unfortunately, no pull carts or walking will be permitted.
All players and scorekeepers must sign the scorecard, and any scorecards that are illegible or missing signatures will be voided, rendering the player ineligible for prizes.
